If that toilet doesn't work, I think it's better to just add disused: prefix to the amenity tag rather than write it out in the description tag.

You added traffic_calming=island area. However, as documented in osm.wiki/Tag%3Atraffic_calming%3Disland this feature is intended for non-pedestrian elements. What you are looking for is crossing island, not traffic island.
