I don't use JOSM, so I don't know what might have caused it. Might the edits have been saved twice?

The only reason I spotted it was because there was a duplicate node on one house. This seems to happen when a computer mouse has a 'moment'

This section of railway way/268317775 is marked as railway=disused, which suggests the rails are still in place, is this correct?

If there is nothing present, it should be marked as railway=abandoned.

Welcome to OSM.

You might look at the values here: railway=* when drawing railways. Realise that if you use, e.g. railway=rail instead of railway=abandoned, that abandoned lines will show as if they are current, which is misleading.

I don't think rail=proposed is recognised, but in using such a tag, that the proposal needs to have solid backing, e.g. by a railway undertaking or local council development plan.

There are quite a few farm tracks marked here, e.g.

I see that you have mapped lots of highway=track in this area. However, all I can see in the aerial photos are stone walls of fields.

A track should look something like the track in the photo here: highway=track

If it is for pedestrians only, like this: highway=path it should be marked highway=path
