I would tend to add street names to the sidewalks as well when applicable, similar to how all roadways of a freeway get the name applied to them.

Ramp junctions happen at the end of the theoretical gore where the node originally was. Placement tags on the motorway indicate that the line is right of the first (leftmost) lane. Please be more detail oriented.

It's best to draw streets in segments that match where street names are likely to change, rather than having multiple streets as part of a single segment. This will take working knowledge of how street names work in the US.
