From my experience, not all floors are occupied, and there are often more floors than clinics (presumably offices?). I also doubt that there would be many "minor" clinics in such a rural remote location. As I said elsewhere, if you are not sure, maybe don't do anything and wait for someone to do a survey?

Imo it's still the original speed limit until you are adjacent to a new speed limit sign. I don't see how emerging lane / yellow strips (actually yellow strips can occur without a ramp so it is even less relevant) make it undefined. It's certainly "before".

I doubt this. It's a `=transition`, meaning it's actually a lane sharing a cross-section with the rest of the highway. The 50 km/h sign would usually start later.
