I extended it into the sea to avoid adding the province relations to every single islet, island or coastline there is. Seemed like more work then if the end-user of the data used a coastline dataset and clip all sea.

Doesn't the province administrations also handle water questions?

The problem with Key:healthcare is that I can't list a hospitals specialities and separate them with a ;-sign because of the 254 character limitation. So I used osm.wiki/Proposed_features/Healthcare_2.0/Specialties for a list of specialities. Although I can easily correct them to just intensive
