Those names were already added to the nodes, you'll want to look at the node history using something like https://osmlab.github.io/osm-deep-history/#/node/5583639651 to see who actually added the name

1. Phone numbers have a very specific format requirement, see phone=*#Usage for details
2. websites need to have http:// or https:// in front of the url
3. while it's not explicitly forbidden, you are the only one in the world to use government=psap, so I'm wondering what that tag means, in case there's a better way to tag it

If you're adding a building inside a building, you need to use building:part=* and not abuse the layer tag. Also, while it's not explicitly forbidden, a standard value for the building:part tag will allow more renderers to know what it is.
