The name change from Graffam to Graffem road seems like a typo that has made it's way into many maps. I updated the point where the name changes to where it crosses the creek, based on what I saw in this census map: http://www2.census.gov/geo/pvs/bas/bas13/st23_me/mcd/m2300511125_casco/BAS13M32300511125_006.pdf

If anyone can confirm that the name doesn't actually change to Graffem they should remove that segment.
